Enrollment is based on space available. No enrolled child or their family will be discriminated against on the basis of age, race, color, sex, creed, handicap, national origin, or ancestry.
Each child is required BY STATE LAW to have a health statement signed by a doctor on file and all immunizations updated. The following forms will need to be returned prior to the first day of attendance or brought with you on the first day:

- Parent’s or Guardian’s State Identification
- Payment Processing Form*
- Subsidy Forms (If Applicable)
- Food Program Annual Enrollment Form*
- Medical or Insurance Certification of Health Examination
- Immunizations
- Tuition Agreement*
- Summary of Licensing Standards*
- SLF Enrollment Contract*
- Video and Picture Consent Forms*
